First of all, Welcome!!
For your first appointment, to save some time at check-in, fill out the new patient form by selecting the link below, Then send us a text, or call to get your appointment scheduled.
Please arrive 15 minutes early to that first appointment.
Please bring with you
- insurance card - if insured
- legal ID card
- list of current medications and allergies
- any x-rays that you had taken in the past year. (have them emailed to us, or we can call to retrieve them for you)

We are IN NETWORK with many insurance plans. including but not limited to
- Delta Dental Premier + and Point of Service (POS) (effective 1/17/26 we are no longer Delta choice or Delta PPO/PDN provider)
- Aetna PPO
- MetLife PDP+ , FEDVIP
- Guardian PPO
- Careington
- HealthChoice
- Blue Cross Blue Shield PPO (excluding federal)
--Due to the vast amount of insurance plans out there, please feel free to call our insurance specialist to verify your exact plan.
We will also work with any of the OUT OF NETWORK plans you may have.
"In Network" -- we have reduced our fees to work with the allowable fee issued by your insurance company.
"Out of Network" -- if there is a difference between our fee and the allowable fee issued by your insurance company, then you are responsible for that difference.

Dr Bowler understands that sometimes you cant get insurance, or its just too expensive, so she has designed a discount plan for our patients that do not have, or cannot get dental insurance. Please call us for details.

We are currently not a provider for Soonercare, Medicare, Medicaid.
